Curious, OS X has not used the bubbled 3D theme for some time. The icons have been distinctly flat. If you are running OS X 10.11.6 did you install some sort of theme pack?

At least with the pre-CC version I own of PS (CS5) if your file has an alpha channel/transparent layer when you use Save As there will be a window with a checklist and the alpha checked. When something like this happens cancel then make a copy of the file and work with the copy. Protect the original or you could make it worse. Can they send you another file? Are you both using the latest version of TVPaint? If they are on a newer version than you they would be able to open your file...
